Definition

Tingling in the context of anxiety is a pins-and-needles sensation, most often in the hands, feet, or around the mouth, produced by the changes in blood chemistry that accompany hyperventilation. It's one of the most distinctive features of an active panic attack and is one of the symptoms that most often sends people to the emergency department.

What it can feel like

A buzzing or prickling in the fingertips. Numbness around the lips. Tingling that climbs up the arms. The sensation can feel alarming, partly because it's associated in popular culture with stroke or heart attack, neither of which is the cause in this pattern.

Why it happens

When breathing speeds up and carbon dioxide falls, blood pH rises and free calcium binding shifts. Peripheral nerves become transiently more excitable, which is felt as tingling. The effect resolves within a few minutes once breathing slows and carbon dioxide returns to normal. New, sustained, or asymmetric numbness or tingling, especially with weakness, slurred speech, or vision changes, deserves urgent medical evaluation, because those features can point to non-anxiety causes that need to be ruled out promptly.

When to see a clinician

Tingling that arrives during clearly anxious moments, particularly with hyperventilation, and resolves as the breathing settles is usually benign. New, persistent, or asymmetric tingling warrants a medical evaluation to rule out neurological causes.
